Common side effects include headaches, abdominal pain and cramping, nausea, vomiting, loss of appetite, rash, fever, rare diarrhea, respiratory infection, and arthralgia.

Avoid balsalazide if you are allergic to medicines containing salicylates, such as aspirin, or mesalamine (Rowasa®, Asacol® HD, Pentasa®, Canasa®, Lialda®, Apriso®, and Delzico®).
