The Qorus community has given me access to resources I might not have found on my own. More importantly, the relationships I’ve been fortunate to build have opened doors for both me as a provider, as well as providing resources and opportunities available to my patients. Collaboration with IBD specialists and professionals nationwide has significantly benefited our clinic, providing insight into different approaches to managing and treating IBD. One of the most impactful changes we’ve implemented as a result is the development of a “high-risk list.” Identifying patients at increased risk for ED utilization and monitoring them more closely has helped reduce ED visits and improve the quality of care we provide.

Joining IBD Qorus has been one of the best decisions we have made as a practice. Being part of this network has allowed us to learn from peers, adopt proven strategies, and grow in ways we simply could not have on our own.
The treat-to-target framework has transformed how we care for our patients. We now order labs before appointments so results can be discussed in real time, open urgent slots to keep patients out of the ER, and provide handouts that empower them to take an active role in their care. We also established a weekly multidisciplinary meeting for our highest-risk patients. Hearing directly from patients about how supported and cared for they feel by our team has been one of the most rewarding parts of this journey.

One of the more valuable changes we made in our practice from QORUS was having a weekly team huddle to discuss and proactively manage our high risk IBD patients. By doing this, we have been able to decrease ER visits and improve access to patients with an acute/urgent issue.
