Despite the availability of advanced therapies for moderate to severe ulcerative colitis (UC), such as Janus kinase (JAK) inhibitors and interleukin (IL)-23 inhibitors, many patients with UC are subjected to repeated courses of steroids, which can prevent them from achieving sustained remission and expose them to unnecessary adverse effects. A lack of standardized collaborative protocols and fragmented communication creates barriers to care. In this Grand Rounds, conducted in collaboration with the Crohn’s and Colitis Foundation, expert faculty will examine structural and practice-related barriers to optimal care and provide practical strategies for replacing systemic steroids with advanced therapies that have a rapid onset of action. Learners will also gain valuable insights into treatment selection, shared decision-making with patients, and setting realistic treatment goals.
Upon completion of this activity, participants will be better able to:
Analyze systematic and practice-related barriers to implementing a treat-to-target strategy
Construct treatment strategies that replace repetitive steroid bridging with a rapid-start systemic alternatives for patients failing conventional therapy
Evaluate patient-specific risk profiles (e.g., CV risk, age, and prior therapy) to facilitate shared decision-making in moving to advanced systemic treatments
