General Donation Questions

 

I would like to mail in a donation, where can I send it?

Can I get a copy of your W-9 Form or your 501(c)(3) letter?

 

The Foundation is a recognized 501(c)(3) organization.  You can download a copy of our IRS designation letter as well as our W9 form.

 


What is your tax ID or EIN?

   

Our tax ID number or EIN is 13-6193105.

 


How can I update my contact information or be removed from your mailing list?

 

To update your information or to be removed from our mailing list, call our membership services line at 646-943-7521 or send an email to [email protected]. Please be sure to include your name and full mailing address.

 


How can I join your email list?

 

You can sign up for regular updates to receive emails about our research news, local and national events, and more.

 

You may unsubscribe from our email list at any time by clicking on the unsubscribe link at the bottom of a recent email, or by sending an email to [email protected].

 


What percentage of donations is used for program services?

 

Of every dollar donated to the Foundation, more than $0.80 is utilized by our mission-critical programs: research, education, and support. The Foundation consistently meets standards established by the Better Business Bureau Wise Giving Alliance as well as other charity watchdog organizations, and recently received a three-star rating from Charity Navigator.

Is my Membership fee/donation tax deductible?

 

All individual and family memberships are completely tax deductible.  Under IRS regulations, professional membership has a different level for deductibility because of the benefits associated with membership. Below is the deductibility for the different membership levels.

Honor/Memorial Donations

 

How do I make a donation in memory or honor of a loved one?

 

You may give a tribute donation in honor or memory of someone or to commemorate a special occasion by:

  • Sending an e-card
  • Making a gift online and requesting a paper card be sent
  • Over the phone, by calling 646-943-7521

If I make a donation in memory or honor of a loved one, how will they be notified?

 

We will send a card to your loved one notifying them of the donation.  If you are mailing a check, please include a note with the name of your honoree, as well as where you would like the card sent.

 


Will the amount be shown on the card if I make a donation in memory or honor of a loved one?

 

We do not disclose the donation amount on the notification that is mailed to the honoree.

 


I made a donation using the general donation form online, but meant it to be in honor/memory of someone. How can I make sure they are notified of the donation?

A family member passed away and I would like to request that donations be made to the Foundation in lieu of flowers.  How can I be notified of any donations?

 

You may set up a personalized memorial page to request donations in lieu of flowers here.

 

You may also call our donor services line 646-943-7521 or send an email to [email protected] to alert them of any potential gifts. We will keep your information on file so that we may send you notification of donations received.

Stock Questions

 

How do I donate stock to the Foundation?

 

You can donate appreciated stocks to us by transferring them into our account. Because electronic transfers often arrive with no donor name attached, we ask that when donating stock you contact the Foundation at 646-943-7425 with your name and the number and type of shares of your donation. Our stock donation page has detailed information on how you can donate stock. 

 


Third Party Fundraisers

 

How can I hold a fundraiser for the Foundation?

 

You can create a personalized fundraising page to mark a birthday, holiday, special event, or to honor a loved one. Tell your story and why you're helping to find cures. Then, ask your friends and family to donate to your page.

Matching Gifts

 

How can I get my donation matched by my company? 

 

Employer-matching gifts are a great way to increase the impact of a donation. Because companies have different policies and instructions for matching employee gifts, please see your Human Resources Department for details. We can verify receipt of your gift and complete the process.

Take Steps

 

How can I make a donation to a Take Steps participant?

 

Donations can be made with a credit card on the participant’s fundraising webpage.  You can search for a participant’s donation page on www.cctakesteps.org.  Checks made payable to the Foundation should be sent to the fundraising participant.

 


How will I receive a tax receipt for my donation in honor of a Take Steps participant?

 

For all donations made online to a participant’s fundraising website, a receipt for the transaction will automatically be emailed to the donor.

 

For offline donations sent to the participant, an acknowledgment letter will be mailed to the donor a few weeks after the donation is submitted.

 


Who can I contact to find more information on participating in a walk?

 

Your local chapter can provide you with more information on participating in a walk and help you set up a personalized fundraising page. You can also visit www.cctakesteps.org for more information.

 


Team Challenge

 

How do I make a donation to a Team Challenge participant?

 

Donations can be made with a credit card on the participant’s fundraising webpage.  You can search for a participant’s donation page on www.ccteamchallenge.org.  Checks made payable to the Foundation should be sent to the fundraising participant.

 


How will I receive a tax receipt for my donation in honor of a Team Challenge participant?

 

For all donations made online to a participant’s fundraising website, a receipt for the transaction will automatically be emailed to the donor.
